I acknowledge that my son assumes all risks of being injured while participating in the 5 Towns Flag Football League and I agree to hold the 5 Towns Flag Football League, it's officials, coordinators and all other staff, harmless from any liability, loss, or personal expense arising out of my son's participation in the league. The cost of League Registration is: $180 (+$10 processing fee for Credit Cards). Checks can be made out to: 5 Towns Flag Football league and Checks should be sent to: 355 Edward Ave Woodmere NY 11598. There will be an option to pay VIA Credit Card if you would like after you submit this form. All participants will receive an official team color jersey which has to be worn to all games.
